Out-patient Claims

In most cases out-patient or dental treatment is paid for by the policyholder. An out-patient claims form is then filled out and submitted to the insurance company along with any receipts. After this process has been successfully completed the insurance company will then reimburse the policyholder after the plans deductibles have been taken into account.
