• 就診前向接待員出示您的保柏會員卡和身份證明作鑑定。
• 簽署索賠申請表確認會診後有接受到治療。
• 離開前解決診所/醫療中心任何共同支付，或免賠額/超額款項。
• 例如，如果您與我們的門診治療政策有20％的共同支付，您離開診所/醫療中心之前應該支付20％的總賬單。在這種情況下，如果您的總賬單是300美元，我們將直接向診所/醫療中心支付240美元，你接受治療後和離開診所/醫療中心之前將支付60美元。
